Sample appeal letter for a denied claim

Use this as a starting point if a claim is denied.

Subject: Appeal of denied claim #[number], model [model], serial [serial]
Hello [Company name],
On [date] I filed claim #[number] for my [appliance], purchased on
[date]. The claim was denied on [date] because [reason given].
I believe the problem is covered because [short explanation, such as
"the failure is a defect in a covered part and I followed the manual"].
Attached are my receipt, photos and the technician's report.
Please review this decision and reply in writing within 14 days.
